    Its pretty sad when employees at this nursing home advise you to take your family member out of this particular facility.

    A friend was sent here for rehab and a lady was on the floor and staff did NOTHING. She was down for 15-20 minutes!!!! When staff was approached, they were rude and did NOT move to get the lady off the floor until they saw me taking pictures. This place needs to be investigated and shut down NOW!!!! Please make other arrangements for your loved ones.

    My mother didn't get adequate care that was needed. Staff was unkind and did not shower her when she requested to have one.Very poor on meals and nutrition. They refused to give my mother a shower after nine days.

    This facility should be closed for neglect of their patients. Please do not put your love ones in this facility. I can not begin to tell you how they neglected my mother's care. She almost died in this place. She is now in the hospital because we demanded she get a higher level of care. She is in bad shape and they did not care as long as they receive their money from you. Please do not put anyone here. Not enough staff, bad management, inexperienced management and staff working too many hours .

    I am very disappointed with this facility and would not recommend SKLD to anyone. After using this facility for respite care 3 times,for my Mom who has advanced dementia, I have experienced nothing but negative issues. The first time my Mom was there, she returned home by ambucab and was soaked in urine from her back down her legs. The staff was very apologetic and assured me that a meeting with the discharge staff would investigate what happened. After speaking with the admission contact, I had her stay there the second month for respite, as they had open beds. There was a lot of miscommunication, I had to make trips there to sign forms that were already signed, and meds were given that I did not authorize. Most recently, after a respite stay in December, she was returned home without her wheelchair, which was clearly marked in several places with her name. I had to make a trip down there to recover it. The next day when I opened her suitcase, I found 3 days of clothing missing, All were clearly marked with her name. When i contacted them I was told this was probably due to the fact that she was in a different unit and they would check this out. I received a call saying they found her nightgown, and would continue to look for the other items. After numerous calls and conversations, I received a call from one of the nurses who said that the clothing was there and I could pick it up. My; mom requires 24/7 care and it is difficult for me to leave the house. My daughter went there 2 days later, however there were still two missing items-they were the most expensive as they included a special fleece jacket with XMAS decorations on it. After more calls, I received a call 3 weeks ago from the nurse saying she would be shopping at Kohls the next day and purchase comparable pieces and would drop them off at my house that night. Lo and Behold, it's been 3 weeks now and I have heard nothing. This place seems to be very mismanage, there is a lack of communication between departments, and the care she received was nothing I would wish on anyone. They are very good on the phone, insuring you that all is fine, she is doing well etc., however, because of COVID, there is no way except to note her condition when she got home to evaluate this.
