DO NOT go here. Seriously.
Their machinery does not work properly. I was told my eyesight was bad, but turns out I have 20/20 and their machine just didn't work. I took a picture of their machine, showed my eye doctor, and she had me read a wall and said that these machines are outdated, and wasn't used properly. I paid for one doctor to tell me this office was wrong, and was scammed out of a pilot medical because of this. Cohen and office decided to scold me on the phone when I politely told them they were wrong and requested they fix what they did.
Fast forward one year. Legally, if a physical was had less than three years ago, I can use the data on a driver's permit application. They decide that this doesn't apply to them, and make up excuses for not giving me my own medical records. The doc is retired, no longer works there - Okay. I just need someone to copy the numbers an verify that I was in for that appointment. The appointment was less than three years ago. The next excuse is that it would have had to been less than a year, which is blatantly false information.
I feel like this office will do anything to lie and take more money from me that they should never have gotten in the first place. Their office staff is sharp-tongued, has no patience or problem solving skills. Be warned.
