      image Did not make sure they had correct patient information Pulled information from a 2 yr old chart. Abandoned patient without medication or anyone to take over care on a weekend forcing several missed doses of necessary medication. Very rude staff. Patient offered to make accommodate and meet at office but obviously they don't care about patients
