image Carle providers have implicit and explicit bias in their charting language that is not supported by evidence based care. They do not provide quality patient care to all patients with equality, equity, and fairness. Their providers in all areas of practice use biased language in their communication and in their charting. To further cause confusion and significant harm to the patient the providers do not chart what has been communicated by patients in the chart, do not chart what has brought the patient in for the visit, do not provide information that is presented during the patient assessment, and furthermore do not address the assessment information. This can be seen in all areas of practice. There is much greater bias in this and the language with female patients and pediatric patients (particularly when the patients’ advocate is female). Carle as a business, Carle administration, and Carle providers have bias towards those with whom identify as female, neurodivergent individuals, individuals with disabilities, children, cultural backgrounds and differences, sexual differences, and race and ethnicity.

Carle provides patient satisfaction surveys to their patients, however these are not anonymous and if a provider isn’t happy with what a patient writes they will contact a patient directly and interrogate the patient as to why this patient wrote what they did, intimidating the patient and bullying the patient. This makes the patient feel unsafe going forward, yet they cannot change providers due to charting that the provider has done and then the limited access in the area and at Carle. While there is a patients relations department are not helpful of patients and do not address any concerns of patients but rather take the information given and provide it to the patients’ so that they can get ahead of ‘the problem.’ Then the patients relations department will provide this same information to the risk management department so that if necessary they can intimidate the patients. This further reduces the trust that patients have with Carle and the providers at Carle. So no matter what a patient does they will not care for a patient nor will they support safe treatment of a patient. Carle is a corporation that is massive and is more concerned about the financial benefits than the care of their patients.

As a patient who has seen providers here in a variety of departments only due to lack of access to other facilities available. I have had experience with all that is above. I am ready to move away from this area just to get away from Carle.

image In my 50 years in this community Carle hasnt changed still they dont care about their patients and they havent advanced their skills in caring for patients and treat patients even more about numbers than patients. I have not found one provider at Carle who actually listens and actually documents what really happens. The medical providers are cold hearted and reckless. I have had witnesses in the room for myself in the past and been a witness for family and friends due to the inaccuracies in documentation. The medical professionals do not care about what harm has been caused. I have never gone to an appointment alone and recommend all to get copies of their medical records because they do not reflect what has actually happened in the appointment. I guess thats what happened when they take things from previous appointment or dont put things in the chart until much later. They do not document assessments, do not document observations of physical symptoms, they do not document the actual symptoms and just put personal opinion not based on actual facts and evidence. They do not like when a person is advocating for patients and will shame the advocate.
This is why all the signs in the clinical rooms that do not allow recording in the rooms because they will not allow patients or patients advocates to speak up or allow evidence of the contradictions. Patient liaisons are not for patients they are there to support providers only and ensure that they cover their own.

image The providers at this clinic are quite disappointing. They are discriminatory, biased, judgmental, disrespectful, demeaning, and dismissive. They don’t respect a patients’ rights nor do they include the patients in their own care. They use language in their charting that denotes their bias and discrimination against patients with no backing, no true assessments, or the actual truth of what conversations occurred. Documentation of things that did not occur or not even the reason a patient is seeing the provider. They do not provide any education to patients regarding the patients’ labs or diagnoses, or signs/symptoms. They don’t want to do anything for patients. This has occurred with providers from family practice medicine and pediatricians. They cut and paste their notes and don’t actually talk to patients or the parents. They don’t allow their pediatric patients to self advocate, though they pretend to, the notes they write show that this is a false sense of safety and security for the pediatric patients. They no longer think of the true safety of patients. Dr. Dionne Smith, Dr. Sean Fullan, Dr. Vista, Dr. Swearingen, and Dr. Conner, just to name a few. As an advocate I’ve had to review documentation and charts and it’s quite concerning the amount of errors in the charting and worse yet the discrimination in the charts.
