The team at Park Avenue cared for my Mom with end stage dementia for over 3 years. My family visited frequently and the level of care she received, as well as the compassion of the staff is top notch. This is a clean facility with activities, and people who genuinely care. Kindness is something you cant teach a person. They either have it or they dont. They also arranged weekly FaceTime chats with my mom and I since I live in Florida. I have no regrets about choosing Park Avenue for my Moms care and want the staff to know how much we appreciate all you did for Velma. You did great. Mom passed on April 3, 2024 just 3 months shy of her 97th birthday. A life well-lived.

Everybody working here needs to pray for their souls for the neglect they are allowing to happen to the most vulnerable of our population. The name should be Park Avenue Hospice. It's a money making factory for humans. I see a therapist today after the horror I witnessed here. If you don't care if your loved one dies and is neglected send em here. Also check the NHAA watchlist this place is on a nursing home watchlist. I question if the positive reviews are real or not. They probably take the patients phone and write the review themselves!! I have to add I did not PICK this facility it was a "miscommunication". Seriously, if your family member is here get them out!!!! There are other options and the copay is 204 a day with Medicare part A after the 20 days at ANY FACILITY. Ahcccs also doesn't matter the facility. There are better SNF for your family member. Don't let them bully you into thinking you don't have other options. Especially the POS social worker Lisette that tried to hinder my dad's transfer with lies. If I would have left my dad in that poor excuse of a rehab he would not be alive today. 6 days and he digressed at this facility because they all are neglectful and abusive. At another facility he progressed because they did THEIR job.

My wife is 86 and broke her ankle. All of the staff were helpful attentive and courteous. The Therapists, and nursing staff were very helpful in giving me tips and suggestions to care for her when she was discharged and sent home. If something happened to me I would definitely choose Park Avenue Health and Rehabilitation Center.
