My child went in for valley fever and was intubated with tPA medication that was supposed to be drained after an hour to help alleviate the liquid in his lungs. The doctor finished the operation and then left down the hallway, while I was in the the waiting room. He had put tape over the release valve and wrote on the tape to release in a hour and of course no one saw it, under the tape, under his gown, and under the blankets. My husband and the nurse found it 13 hours later after the medication and liquid was in his lungs for 12 hours longer than supposed to be! The only reason we found it, then, was because nothing had drained out of his lungs into the measure bin. The amount of liquid that came out when they opened the valve was astonishing!!! We were extremely worried about the side effects for our son. PCH was terrible about admitting it had made errors and I had to be adamant about them putting it in his record. ADHS investigated and found them at fault for deficiencies in the handling of my son's operation. They do not post deficiencies found like they say they will on the azcheck site, so I will post here. PCH and doctors are just people and can make careless mistakes. Do not put your trust in people or facilities expecting them to do the right thing.
