I highly recommend avoiding Dr. Jacobsen unless you like having your time wasted.
Went here to get a routine driver's license form signed and he refused to sign. When I asked why he refused, he wouldn't tell me. After this experience, I left the practice.
However, it wasn't the first time I received substandard care at this location. I suffered from a sinus infection for 8 weeks and had to see him twice before he agreed to prescribe Azithromicin. As soon as I started taking the antibiotics, my infection immediately cleared up. I told him that I did not have allergies or need allergy medicine, but he tried to force them on me. For 8 weeks, I had to walk around completely congested.
Dr. Jacobsen is arrogant, rude, and the practice is staffed by morons. My parents also wound up leaving the practice because when one of the doctors retired or moved elsewhere, they were told they could not see the other doctor at the practice because they don't allow people to switch doctors.
