My mother was on hospice with Transitions for 1 week prior to passing. Although my sister was her primary caregiver and a BSN, at the 11th hour when we were all emotional and physically fatigued we had relief immediately. Not only did the Nurse show great compassion, she was able to get mom comfortable and we were supported until she passed. The Transitions team are some of the most compassionate, dedicated, committed individuals I have ever met. They showed up when we needed them the most, and I was never concerned about them doing the right thing.

Cannot say enough wonderful things about the team that has taken care of my father n law at our home South East outside of Indy. During his last 5 months and now thru his final days they have provided warm and loving care. Transitions made sure my husband and I were doing ok as well as their actual patient. They definitively treated all of us. And now at final days I get to see how they work together as this intertwined team to provide care around the clock. Truly a team effort and all members of the team appreciate one another.

These caring individuals are amazing. Because of them, I was able to care for my mom and she was able to pass in her own home as comfortably as possible. They were patient, knowledgeable, and kind. They helped the entire family through this hideous process and not just my mom. They're true angels.
