        Not very pleased with this place's policy for new patients.... They want to you fill-out a 14 page questionnaire about your entire history with all you personal data, before they even discuss appointment availability. So if you decide the appointment is too far out, now your entire life is in their hands, and they were never your doctors at all.... They should tell you what available appointments they have available first, and then if it works for you, require that you return the 14 page document ASAP before they can actually book it....
