My mother lives at the Bonaventure and has been there 4+ years. The staff has done a great job caring for her and assisting her when I cannot. I also greatly appreciate Dir. Shelby Lucero who can get things done in all sorts of ways.

Had dealings with this facility after becoming a POA for a friend. The POA was last minute so I had to learn quick how to navigate POA rights and resident care. Right away I had Bonaventure trying to pressure me into setting up automatic withdrawals from my friends checking account. This was a huge red flag since I noticed discrepancies in billing. When I asked the billing department, no one could adequately explain the mystery increase of charges. They also could not offer an itemized bill. Their explanation was only that care of my friend had changed and that my friend was assessed at needing more care. I stated that she had only been under their care for 3 weeks and that the increase of care was not based on medical-since my friend had not been seen by their in house physician. I started to document all further communication via email. Admin could not explain the charges and then would not answer of my requests so I contacted corporate and spoke with their District manager. He was able to explain some of the charges but stated that the assessment of care should not have changed that quickly according to Colorado Law. He refunded a portion of the money. I got into contact with APS and asked for a investigation of the facility. It was only after my friend fell and broke her hip did APS get involved. My friend was transported to Parkview by the facility nurse. My friend's medical POA and myself were not notified by Bonaventure. My friend was pretty much dropped off at Parkview without any notification to anyone including Parkview. A nurse at Parkview contacted the medical POA only after going through my friends phone. Her surgery on her hip was delayed because Bonaventure did not contact the POAs. I contacted the State of Colorado that oversees these types of facilities and filed a grievance for neglect. I also contacted the PPD- SVU and filed a report for elder abuse and theft of monies. I reached out to the ombudsman for further references. After 19 days in dealing with Bonaventure, I gave a notice to vacate. My friend is at another facility and is doing much better. My advice is to document everything with this place and if needed go above the admin that oversees this establishment. Utilize resources-ombudsmen, APS, PPD-SVU and the State of Colorado. Demand to have itemized statements, monthly MARs and a schedule of in-house services. Never allow them access to automatic withdrawals. They work for you. You pay them money for services. One more thing- I noticed a review by one of the Admin staff saying all these good things about this place. This is a conflict of interest. If you look at the majority of their "5 star" posts, they all come from employees. I flagged that post and the others and decided to write this honest review as a warning to anyone deciding to use this facility.

If you are thinking about placing a loved one in Bonaventure DON'T. We have had experience with 3 different care facilities between my mom and dad. Bonaventure was the worst by far both from a cost and a quality of care standpoint. The following information was provided to Bonaventure so that they would have an opportunity to do a factual review of the events. They never replied. During the course of my mothers stay at Bonaventure we have had a number is issues we feel have been caused by staff at Bonaventure. The 3 most important issues are as follows: 1. Loss of bank account information resulting in theft from my mom's account 2. Theft of a check (reportedly from an employee) that resulted in a loss from my moms account. 3. After moving out, the refund of monies was inappropriate. We did not receive a full refund and were charged for items that we feel where not caused by my mother. Details of the events are as follows: 1. My mom was signed up for auto pay. The facility required a blank check to establish auto pay. This was the only ACH my mom was making and she was not using checks for anything else at the time. I went to the facility to review the situation with staff. At that time I was walked into an unlocked file cabinet in an unlocked office where the resident files were maintained. In these files were the blank checks of everyone that was on auto pay. Because of this complete lack of security we cancelled auto pay. We believe strongly that this is where my mothers bank account information was stolen from. It took over 2 months for funds to be recovered from the bank. 2. A bank check, for a monthly payment, was sent to the facility. That check was then redirected to my mom's room in memory care. Note, my mother was incapable of dealing with mail. From there it was stolen, doctored and cashed. It was reported to the police and Adult protective services. Ultimately it was believed that a staff member stole the check. Once again it took over 2 months for funds to be recovered from the bank. 3. My mother passed on 9/7/2023. 9/8 we cleared out her belongings (some of which we donated to the facility). We confirmed with staff, on site, that we were done. We were not told of any issues. 9/29 we started following up on the status of a refund ~ 10/6 we received the refund check minus $1954.30 We requested a complete accounting of the refund as we were not told why the check was short funds. 11/8 left message with staff following up on the status of receiving accounting information on the refund. 11/13 left a message for staff, again, following up on the status of getting the accounting. At this point we informed staff we would be following up Adult protective services. 11/13 we received an email from staff with a very basic accounting. In the email they indicated would call us back later that day. We responded clarifying that in addition to the accounting we would want information supporting the charge (age of the floor, pictures etc) 11/13 Staff called back with inadequate information. I reiterated the need for the supporting information on the charges. 11/14 Staff called back indicating that they had no additional information. We have been ghosted ever since. Update. We received the balance of the refund the end of Jan

I cant say enough good things about Bonaventure Pueblo! Shelby and Carlee, the directors, have gone above and beyond to make my Mom happy and comfortable there after she had to suddenly move from her home of 40 years. After my mom was mugged right in front of her house, we knew it was time she went to a safe place. Since none of us kids live there, Shelby and Carlee personally went and got her and took her in so she would be safe after having her keys stolen. It has been a very hard transition for my mom who is 88 and extremely set in her ways, but they have done absolutely everything in their power to make her happy in her new home. And her apartment is beautiful! Chris, head of maintenance, helped so much and seems to really care about the people there. Even had a cute nickname for mom after a week there. So sweet! The food is very good, and we were there to try all 3 meals. Wait staff was very friendly. Common areas are nicely decorated and clean. I have only good things to say about this place. Mom is finally starting to get used to the changes and I think she will be very happy there. I know I am incredibly thankful that she is there! Thank you Shelby and Carlee for taking such good care of my mom!
