St Ann Health and Rehabilitation Center is a great facility. Facility leadership is dedicated to the residents and goes above and beyond for them. Highly recommended this center.

Do not send your loved ones to this horrific facility. The nurses have zero empathy for their patients. My dad was left to sit in his urine and feces for several hours. He was told to walk himself to the bathroom knowing darn well that he needs assistance. This happened several times. We complained and nothing was done. The OT was kind and extremely helpful. The nurses just push pills. Again, if you care for your loved one, do not send them here. This place should be shut down.

I am having nightmares still from the experience my Mommy had 9/26 - 9/27 at this facility. The Administrator & Director Of Nursing still has yet to contact me. I am totally afraid for the residents that are in that nursing facility right now. My mom literally was left in the cold with a window open all night. In the midst of a cold thunderstorm. She arrived and no special mattress was there. Them knowing that she was supposed to have a special mattress. Her meds were missing, so she had no meds for over 24 hours, a comforter top with dried urine was in the closet, and it just smelled so bad. I could go on and on about this place and how my mom experienced this. Just unreal

Needs to be closed down residents call lights not being answered. Filthy smelly rooms intense urine smell sheets and covers full of dried urine. Not enough staff was the excuse staff used when resident was left in urine all night until 12noon the next day. A worker saying she came to work there because her mother was admitted there and then abandoned residents to attend to strictly her. Sitting around on cell phones instead of answering call lights. Resident left without a proper air mattress, no meds, and left in urine & bowel movement all night. Memory Care patient entering room of bed bound resident. Staff was not attentive to the needs of patients this place should be shut down immediately.
