Save your money and look for a different orthodontist if you're self pay, need a payment plan and don't want to give your SSN. They require a SSN in order to be put on a payment plan, I asked for one because they don't take my insurance. I've gone through another one of my children having braces and her Dr allowed a payment plan and he didn't need a SSN (unfortunately, we moved and he's too far, else I would've gone back), we just had to pay the monthly installments on time before her next appointment. Shop around if you're in this type situation.

Back in January my daughter visited the dentist for her second time for a cleaning. The Dentist noted that our daughter had some cavities and suggested a few fillings as well as a crown. Fast Forward to May, my wife scheduled an appointment to get the fillings done. The front desk noted that the dr. had suggested a crown and my wife politely declined but said we would bring her in for the fillings. The front desk noted we "signed a document. The document was the recommended treatment from Feb which I told them I needed to discuss with my wife, and we would call back to schedule anything. The Day of the appointment, the front office called my wife and "Canceled" our appointment because we declined to put a crown on a 5-year-old. My Wife stayed home from work and kept our child out of school for a scheduled appointment to simply cancel on the day of the appointment after making changes to accommodate it. The Canceling of the appoint the day of is highly unprofessional, especially considering the reasoning was that we were declining a portion of the suggested treatment.
