License number: 0002055982 Do your research and due diligence before you place your loved one in an assisted living facility. I wish I had researched more before placing my relative at this facility. The time my relative spent here was traumatizing, odd, and eventually criminal. The behavior of the owner after my relative moved was childish and despicable. To this day, I wonder how someone in the health profession could be so mean and spiteful and call themselves a nurse. The rights of my loved one was violated in multiple criminal ways. This facility stayed under the radar of many state over site agencies because it was only licensed to have 3 residents. This is a place I wouldn't even send a dog to live and the only reason why I posted 1 star instead of zero stars is because google forces you to click at least one star in order to post a review.

    This facility provides decent care but if you place your loved one here be aware that the rules change frequently and are not communicated well; the rates will change each month; your loved one will be medicated and put to bed before 5pm which limits visiting after most folks get off from work; and, the family paying for the service is disrespected and treated unprofessional when questions are asked about the care being provided. The facility "shows" well but remember all that glitters isn't gold. This place is a joke and after my original review they posted a 5 star google review claiming my loved one passed away shortly after removing her from this environment is completely FALSE. I stand by my comment decent care is provided, my loved one's health did improve during the 4 months here; however, changing of the rules, medication times preventing visitation at work, and general disrespect is why I would never recommend this facility. Lastly, my loved one did not like living here.

    We apologize in advance as licensed professional, we recognize not all families will be happy, but our reputation for providing excellent care has never been in question. Our clients have not only graduated from Hospice, gained weight, advanced in smoking cessation, depression as well had a better quality of life while under our care, even with a Failure To Thrive diagnosis. If our medical and nursing approach does not fit the micro managing family,'s we make no apologies. Our style of care depicts the oath taken "To Do No Harm". This Residential Assisted Living has never disrespected any loved one or family member but has been berated by the Williams' family subsequently due to the inability to change our policues and procedures, upon their loved one's discharge into the Williams' care, the multifaceted, multi -diagnosis client passed shortly thereafter, because their prejudices knew best.

    My husband, Harry Costello, (passed away over two years ago) received excellent care at the resident of Epiphany Assisted Living. The family observed the professional, efficient, compassion, fun-loving attention, given to Harry, as if he were a family member. Field trips - Harry's birthday - were well planned and good family fun. On one occasion, when Harry exhibited signs of un-wellness, the Epiphany staff arrived at the ER, ahead of the family. Would you view this as true, Christian care - Oh yes! May God continue to bless the Patterson family, as they serve God through their Epiphany Ministry.
