Thank you for your review. I am sorry you were not satisfied with the initial intake/questionnaire. It is our standard form and after discussing your hesitancy to provide sensitive information, you were told to leave it blank and return the form without your sensitive information. Also, when a client makes an appointment for a either an in-office visit or a telephone conference, it is customary for the client to initiate the call and/or come to the office. You failed to keep your appointment by not calling in at your scheduled time.I hope this clarifies the misunderstanding. Again, I'm sorry to hear that you were disappointed in our service.
