        Not a Dr office just a chiropractor. I Was told they had a Dr on site when I scheduled an appointment. Upon arrival they said it's just a chiropractor.

      Sir, When you called you asked if we have a doctor on site. Chiropractors are doctors of Chiropractic. Your form is very specific and requires a medical doctor only. Nurse practitioners, Physician assistants and Doctors of Chiropractic are not approved, per the TDLR. I am sorry we could not have helped you but when you called you did not ask if we had a medical doctor on staff, just a doctor, which would be the chiropractor. Therefore, I am confused by your 2 star review when we gave you clarity to your situation.
