If I can give them a triple quadruple zero I would. When I tell you the care there sucks I even had a staff member come in and close the door and tell me and my brother both that you do not want him in here they don't care they're not going to take good care of him. She was exactly right they kept him in a diaper all day long soiled his butt was raw. They left him in a chair for almost 12 hours I will be contacting the necessary people I have names pictures and videos. The director Brenda is a very nasty person she was very rude to my sister on the phone who is a senior citizen with Parkinson's and heart failure. One nurse didn't even give him his medicine until about 1:15 in the afternoon when he was supposed to get it first thing in the morning she stayed in the same spot for 2 hours I have her on video. They brought no snacks for the patients after dinner my brother didn't have anything to eat from 6p until the breakfast tray came the following morning. When we found this out we brought snacks for him. I've never seen a team of medical staff members that don't give a darn about nothing but a paycheck they need to be shutdown. I called him the other day to check on him because I live out of state he said he was so sick and the nurse never came back in. I called the front desk to call 911he gets to the hospital he has COVID and his arm is infected from a procedure he had done and he needed emergency dialyzation which they ignored all the fluid on him. His roommate complained everyday to the nurse manager nothing was done DO NOT LET YOUR LOVE ONE go there ever. shame on all of you so called health care workers at Signature

If I could leave no stars I would.. this is the most nastiest facility I've ever seen almost all the patients have open wounds and bed sores they are left in there on feces for hours while the cnas walk around on there cell phone's or just sitting around they don't shower any of the patients they leave patients sitting in wheelchairs for 12 hours or more and tell them they have to wait for the next shift to put them to bed.. There's this one nurse Wanda gobley that uses restraints on patients when they don't stay in there wheelchair she ties them down to the chair and whenever they don't wanna to sleep she gives them melatonin or double medicine . The charge nurse Monica for night shift leaves the facility unattended for hours and the LPN has to look for her... All the cnas and nurses walk around eating the diabetic patients supplement snacks and when they say there hungery they tell them that the kitchen staff didn't leave any supplements this facility should be shut down

I work with Primacy at time at discharge to assist their patients going home. They have a great team and communicate effectively.
