Lebanon Skilled Nursing and Rehabilitation Center

this is his first time in this ECF/Rehab facility and so far he has no complaints. I spoke with several of the care staff and if their attitudes are always like they were when I visited, I'll have no problem or complaints. As my husband's advocate, I can be very critical at times. But so far, so good.

My father had a horrible 3 day stay he rang his bell, he needed to go to the bathroom it took them over 45 minutes to answer his bell. Then they never came back to get him off the toilet I got him off the toilet. Then they sent him to the ER because he became unresponsive because they never put his CPAP on him while he slept. Very understaffed a terrible place I wouldn't recommend this place to anyone.

Worst place. Dirty, I have pictures to proof it. Blood all over chairs in patients room. I have to go there everyday to visit my brother and the nurses are unprofessional, over worked and have no knowledge on how to treat patient and family. Rude is putting slightly. A whole week as gone by and my brother has not even been bathed. They had no idea where his meds were. Hes diabetic and they are giving him everything that he should not be eating because they dont know anything about anything. Yesterday one of the nurses comes into the room and tells me to tell my family to shut up in a very rude manner. The other day just because we asked about his care they nurse had a fit, throw her mask and started screaming like a mental person. I could go in and on. Its a horrible place to have a loved one in. Oh I asked for a supervisor and the nurse that had the breakdown is the supervisor. Uffffff God have mercy. The beds are so old my brother cant even sleep on it. This place needs to be renovated, more staff and a complete re.training of staff, or shut down by the health department for the horrible non care their patients.
