    The worst facility on the planet! If you want your loved ones cared for, dont send them there. The nurses & aides hide the call bell remotes so the patients cant reach them when they need something. They will let the phone ring 30 times before they pick up and are very annoyed with you when they do answer. Visitors have to hunt for someone to let you out when leaving because no one is at the desk and there is no code posted anywhere. They get government funds to do a piss poor job. Most of the staff are rude, unfriendly & unprofessional from the head nurse on down to the front desk clerk. My sister was only there for 2 weeks and got severe bed sores because they left her in bed all day. That halted her rehab progress severely. Terrible service.

    I was placed at Evergreen for an extended time in early 2023 for rehabilitation for an injury. During my stay of several months, I felt very comfortable there. The day nurse, Donna Davis, is one incredible individual. She is very knowledgeable and caring of her patients and will speak up for them if they cannot. She personally saw to it that my wounds were properly cleaned and wrapped and that I always received my medication on time. And if I ever needed anything, she would find a way to make it happen even if she was taking on several other tasks at the same time. I don't think I could have made it thru without her help.

    Evergreen Crossing &Loft have terrible CNAs they never assist the patients leave them for hrs even when their light is on for help. This man had not been able to eat for 5- days.He asked for tums for 2- weeks and was never given any causing him to not be able to eat. They do not change the people who need changed or turned. They sit on their phones and leave those poor people to suffer. NEVER PUT YOUR FAMILY IN THAT FACILITY
