AVOID THIS FACILITY IF YOU CARE ABOUT YOUR LOVED ONE. DISCHARGE PLANNING CATASTROPHY. Chose this facility for my significant other following hospitalization. Initially the site appeared clean, well staffed, ect, however, after a few visits, I noticed there was never soap in the dispenser in the patients room, which led me to question how staff was washing their hands between patients. Never an answer, never any soap, and during the duration of stay, my loved one kept contracting gastro intestinal infections. This, by far, is not the worst of it. For several weeks prior to discharge, I attempted to communicate with Caroline Luedeking, social worker, regarding discharge planning, eg equipment and supplies needed, home care, transportation, medical orders, and received no answers that made any sense, when she bothered to return calls. Finally had to reach out to administration, who in turn had the social services director reach out. I was assured that everything would be in place by discharge. Love one discharged the Saturday before Easter with nothing, NOTHING, in place. I'm writing this review exactly one week following discharge and still have none of the required supplies. I've tracked down the problem and apparently Warren Barr changed her Medicare/Medicaid status to long term care resident, hindering us from receiving supplies/medications at home without paying out of pocket. Was informed by Medicaid that it will take several weeks to correct. This clearly falls in Caroline Luedeking, this could and should have been corrected weeks ago.

This place is not what you would expect for the "Gold Coast". The rooms are in terrible disrepair. The staff is rude. The number listed for 24hr reception is never answered. You can't contact anyone at this facility to inquire about your loved ones

Do NOT send anyone to this facility. This place should be shut down. Ashley, who was my Mom's case worker would NEVER respond or call back. We understand she has other patients but there is NO excuse for not even a response. We were just ignored by her. They didn't follow Dr's orders and she didn't receive all of her medicines. Her therapists that worked with her were great and few nurses but the overall experience was horrible.
