I worked extensively with Pat Gray-Smith, APRN, BC, FNP/ANP, while I was employed as a Nursing Director in various ALF communities. She and her assistant, Andrea, always went above and beyond to ensure our elderly residents received the absolute best care possible. From routine physical exams TO med mgmt TO mobile radiology TO urgent issues TO lab collection TO psychiatric care and even showering or shaving difficult residents, all patient care was always provided on-site. Pat has a huge heart and a unique way of connecting with the senior population, especially those with dementia. There is honestly no better senior care provider in the area!

Perfect for Elderly family members on medications , and they are fast on ordering senior in home care , and even lab work at home . Nothing compares in the Atlanta area. Always caring and helpful.

Horizons Healthcare helped my mother to walk again after a lengthy stay in the hospital. They worked with her until she could walk on her own again, which allowed her to have her independence back. I hope my mother doesn't need rehab again, but if she does... I will request you again myself! Thank you Horizons Healthcare!

After moving from out of state, I was looking for a primary healthcare provider and my insurance plan recommended this office. Its a quick 12 min drive from home. My 1st visit went well, office staff smiling , helpful and kind, absolutely no feeling of being rushed at all. The premises are prestine in cleanliness. Saw Dr Allen and his calm demeanour shines. He spends time in getting to know you and infuses patient education on almost every subject detail as if you are the only one to see for the day. He truly embodies the old school type of medical practice yet his knowledge is just as up to date with modern medicine. I have no doubt the office NPs are as equally professional and attentive. I hope to form a lasting primary care relationship going forward.
