We are sorry you are upset, after a thorough consultation regarding your hair history and current goals we were concerned with the integrity of your hair so we decided to do a strand test to see how your hair would react to the process which would be necessary to reach the desired results.  This process was done at no cost to you and takes a bit of time. The hair did not lift well due to previously used box color.  You were informed of this and wanted to proceed anyway. This is why we wanted a high lift color release form, they are used to protect our stylist from legal issues resulting from clients that refuse to take our professional opinion into consideration and want to move forward with service anyways.  As home color is unpredictable and we did not believe you would be happy with the results we were not comfortable moving forward without the release.  Unfortunately we were having issues getting the form to print.  We apologize for the issue with our printer. We take pride in what we do and have 50+ years combined experience.  It was our professional opinion that we could not achieve the desired bright white hair color you were requesting without irreversible damage and we explained that to you multiple times.  Before you left you called us unprofessional and told us we had wasted your time but the truth of it is that we were as professional as possible, we performed a strand test with product that we had to pay for and lost money by telling you no.  No stylist wants to lose a 4 hour time spot and waste products and time.  I'm sure you were able to find a stylist that was willing to take the chance of over processing your hair for their pockets to benefit.  Clearly we are not the right salon for you.  Best of luck on your hair journey.
