This facility along with it's case  managers are quite disengaged when it comes to managing their client's cases. There seems to be little to no pro-active action taken by the case managers regarding their client needs or concerns. I find this to be not only frustrating  also very disappointing for everyone involved. As a parent of a child who's case is currently being managed by supported living services on tunnel st, in santa maria ca.  I can tell you we are definately concerned with the lack of progress or interest for such shown by this facilities case managers, and as such are seeking a new alternative / facility for our daughters mental health and wellness care.

Transitions handles mental health services for my relative with schizophrenia living in S. Maria. I have to echo the earlier reviewer who said Transitions is "disengaged" from the clients they are supposed to be supporting. The whole point of Transitions and the Supported Housing Program is to help seriously mentally ill people remain in their own homes. They are supposed to do this by helping them to take their meds, providing case management and visits with appropriate counselors and other mental health and medical professionals. Almost none of this is happening with my relative. I don't know if they're understaffed, uncaring, or just lazy. As of today, my relative is spiraling down, talking to "demons" she sees and hears, not remembering to take her meds due to MEMORY issues probably stemming from physical illness and the many meds she takes (that Transitions is seemingly unaware of because they refuse to take the complaints of her care provider seriously) and because they don't ever seem to come out to the house to see her. I called and requested a medication review over a month ago and was told I'd get a call back. It never happened. They tell her care provider that my relative has the right to refuse her meds. Well, she's NOT refusing them, she's forgetting to take them, which is a completely different thing and now she's going off the rails. I consider them responsible for the slow imploding of my relatives mental health.
