        Would not suggest this irresponsible place for pt. They cannot properly collect Insurance info over the phone. I went in filled out all the initial paperwork provided a copy of my insurance card because they took it down incorrectly over the phone. Weeks later they never verified my insurance info and lost paperwork. I was told yesterday that never was scanned into my patient profile. So they lost my PHI, I asked to speak to a Manager and was told there was only 1 person on site.
