Because that is my grandmother which I have proof of I am the granddaughter. So therefore I have a right to show anybody all the pictures of my grandmother and how she looked until the day she died right after we yanked her out of there. She went straight to hospice. She didn't go anywhere else. She was deteriorating here and was on hospice.As soon as we got her there, she went straight into husband's care.I'll show pictures of everything AND ALL OTHERS REPORT GRANDMA was in 2023 horrorable.SHE DIED 2 MONTHS AFTER WE GOT HER OUT OF THERE ALL ARE NEGLECT & ALL PACHENTS WERE neglection / AND WARNING WORST PLACE sept im REPORT TO THE STATE ..THEY NEGLECT PACHENTS MY GRANDMA AGE 86 HORABLE This place need to be shut down place, it is a nightmare it caused my mother. My family trauma, we can't stop thinking about it mentally. And emotionally, MY MOTHER IS DYING SHE IS AT HOME ON HOSPICS I'm going all the way to the top to attorney. Genera neglected PACHENTS grandmother was put in this place After hospital .in ERROR SHE DIDN'T NEED REHAB SHE WAS Dementia stage seven . My grandmother was neglected. In this place and all other pachents are neglected left aloneI was there everyday AM2 PM we took shifts we'd never left her alone. Everything I heard everything I saw I have pictures of everything and it's legal my grandmother got abuse which is neglect and all the other patients are neglect everybody is left in the room and they never check up on them. I saw patients with the door is open, falling half off the bed. In the room, not once did anybody check up on them? Can you imagine the bed source losing weight? Whatever it is, they never came inever. Asked them for anything or check up on them. Or water or anything they walk in. Put a plate, a tray, and walk out. It's not even food, they're losing 7 pounds a week. My mother is dying faster. She wasn't like that when she got in. It's a horrible place, the whole intire building FALLING apart, and a glad they hang up on you. They don't answer you back, nobody's around their rude disrespect. They always hang up, they don't answer their phone I am going all the way to the top to every single Turney general you name it and I have proof there is a woman who verbally abuses you if you ask questions on your family and her name is Jennifer I have a last name. Cause there is a couple of them, this is traumatizing my family when my mom went in there, she weighed a 119 now when we got her out, she weighed a 109 she lost 7 pounds per week. I have all the evidence my family does too, where very well known in the community, we're very large, we are in the ministry. She wasn't supposed to be here, that's why we had to hurry up and PULL HER OUT right now she's dying faster. NEGLECT MY LOST 7 POUNDS EACH WEEK GRANDMA. THEY DO NOT CK ON PACHENTS. DO NOT CARE DO NOT TAKE CARE OF THEM DO NOT FEED THEM DO NOT GIVE THEM FOOD that's a joke frozen gross cold it's LIKE FROM THE DOLLAR TREE. YOU CALL THEY HANG UP ALL YOU GET TRANSFER.NO ONE ANSWERS THEY HANG UP

Horizon as I have seen and experienced really does care about all their patients. They do everything to answer call lights in a timely fashion and help anyone who needs it. Ive seen patience and compassion when sometimes not the easiest situation. all the workers from kitchen to admission, nurses and CNAs have passion for their work and knows it matters to their patients. They make it as easily as possible for everyone and do their best to make time for all there patients even when their is a lot per section. I would send any loved one, friend or even acquaintance here because I trust the staff fully. As Ive seen a lot of workers having their family members getting help here already

Much like any care facility, there are wonderful nurses and CNA's that give great care, and the others that are horrible. This facility is no different, and I have witnessed both. The turnover is terrible and some nurses will turn a blind eye to patients illegal activity. The sad thing is, the good staff members suffer with those who choose to do nothing, and the good ultimately get blamed. I'm surprised they are still open. State really needs to keep an eye on this place.

Holly was truly amazing. She went above and beyond to make our transition smooth and was incredibly attentive to our needs. Throughout our stay, she checked in with us daily to ensure that everything was taken care of. She was always readily available whenever we needed her. The place was consistently clean and our entire family, including my toddler, felt welcomed and comfortable. One aspect that I particularly appreciated was that even our furry family members were allowed to visit as long as their vaccines were up to date. I highly recommend this location, as it is situated in a peaceful and safe neighborhood.
