        First visit with Dr Micheal P , on 10/4 was encouraging for me;But !! Yes BUT ! I had appointments for 10/12, Mammogram @130,Dexascaan/Bine Density @ 330p,Chest X-ray . No orders ,after 7 days had been sent for Mammogram nor Chest X-ray!! Well after couple of hours finally got Mammogram done!! BUT NO CHEST X-ray orders, Rep from SMH called several times and finally they sent over, BUT WRONG PERSON!! Spoke with Brook who was rude and snappy, .Well today,10/13 returned and still NO CHEST XRay Orders!! YES NO CHEST XRAY ORDERS!! Took several hours , and I called myself,Brook at Sage on Airport Blvd,old Catos building! I am really concerned as to what why this occurred 2 days in a row and 7-8 days after appointment should have been made! I need a genuine answer

We will be calling you today to resolve the matter.Update: We understand that our care coordinator did not fax your order to the correct fax number, and that this has been corrected to avoid inconvenience going forward. Care coordinators have also been instructed to call the provider to confirm they received the order. I hope you have since been able to complete this diagnostic work. Are you satisfied that we resolved the issue for you?
