image Advanced Subacute must be the cleanest healthcare facility Ive ever been in and I was an RN for 38 years. The staff is generous, accommodating, and responsive to my needs. Im especially appreciative of the nurses, aides, and auxiliary staff. Physical therapy challenges you and they are very pleasant people to work with. The gym equipment is extensive and their hard work with me has shown in my recovery. I highly recommend this facility.

image My experience with this facility has been five star top-notch care for my mother. My mother was admitted into the rehab center after having a partial hip replacement. I cant say enough about the nursing and aid staff as well as the management staff here as well. I want to give a big shout out to Dean, Myra, and Leola for all of the amazing care and time they have given to my mom to bring her stay. All of the other nurses and aids have been amazing as well, Im sorry if I didnt get all of your names, but you all deserve a shout out. Highly recommend this facility to anyone who needs care. They are very attentive, and the facility is kept extremely clean and tidy. I cant say enough good things about the experience so far.

image The staff has been very nice and helpful with all my needs. I was originally supposed to be here for about 3 weeks but the rehab has been excellent and that was cut down to 8 days. My meals have been average but whenever I had an issue they were quick to fix it. I really enjoyed the burgers!

image Writing this review is very painful for me. Thats because I am an RN who has practiced bedside nursing for 45 years and Im sad to say that my husband who has a t terminal muscle disease complicated with other medical issues has required rehab visits due to weakness
following hospitalizations. Otherwise I take care of him at home 24 hours a day.
We came to this facility from another one due to very poor care.
We came hoping to finally receive the necessary care my husband needed.
On all levels this faculty failed my husband except for the therapy which was sparsely provided.
Never mind they dont clean the rooms or empty trash on a regular basis. They gave no supplies available for the patients including soaps /wipes/linen/underpads /pullsheetsEtc. I have never seen anything like it.
My husband experienced disrespect/negligence. He was challenged as to why he couldnt do something. He was left in urine and feces for different time period s. He had to hear from the staff that they were from an agency and didnt work there. He was told to just pee in a diaper or have a bowel movement and they would clean him up. They do not answer call lights in a timely manner and they sit at a nurses station where the call light system does not work. They told my husband and I that we were a problem. This includes the evening supervisor talking negatively regarding my husband to several staff members. Hes was then a target and we felt few wanted to care for him.
I spoke directly on several occasions
to the on site Administrator a Director of nursing without any effect or improvement All my concerns were clearly brought to their attention. On one of our final days there I asked a gentleman in hallway who stated he was in charge of patient relations my concerns.
I decided to bring my husband home where he was so weak he suffered 2 falls after 6 weeks of rehab. I was told by therapy he was stable to go home.
Im just sick at the reality there are places you send your loved ones for help and care and there is neglect and mistreatment /lack of professionalism and no accountability.
Please do not send your loved one to this facility. I only captured some of the issues we experienced. Ty

image Zero communication.
Same voice called me as admin as well as social worker.
EVERY time I saw my mom she was
A mess and only 2 showers in 4 weeks. She went to get strong to use her walker and I forgot to bring her shoes....they never once asked for them..I called mom's insurance Co to report what I found and then I was asked to make a 911 call to take her back to the hospital.
Do not take anyone there as they are full of excuses and stories.
I'm being very nice on this review.
