Please educate yourself about this facility before having your loved one admitted. A huge lack of compassion by staff, unclean and no attention to the needs of a patient for a full night. Mom was only there for one night- she checked herself out the next morning. Truly, this is a facility that the state should look at thoroughly. I gave it one star only because I needed to put down a rating.

There is a nice front porch. No smell in the facility. Therapy was good. The staff were nice.

My grandmother passed away recently at this facility. After retrieving her mefical records, I saw this facility was at fault for numerous falls she had, numerous sepsis infections that were treated at nearby hospital, that she had contracted at Avante. She made numerous reports of abuse but because she had dementia, they were IGNORED. If you love and truly love your loved one, avoid this facility at all cost! I could not make choices for grandmother because of ignorant and selfish family members that had control. I will however do what I can to file an investigation and shut this facility down for neglect.

I think the staff is caring and helpful. The physical therapy team has made a lot of progress with my mom.
