        Not elderly friendly receptionist. She asked that I fill out an online form and I told her it would be challenging for me. She responded by telling me I could not see my provider without the forms! I asked for the manager and she appeared with a scowl on her face, opened a desk drawer and grudgingly pull out a paper form then turned and walked away. I plan on changing my care to the Ballard office unless they share same manager.
