InnovAge Pennsylvania LIFE - Henry Avenue Center

The clinic does not communicate with clients family when needed. I have been waiting for Dr. Tidus for over a month to complete a Physicians form and send it to my PA enrollment broker. Every time I call there is an excuse, first we were faxing the wrong number (though it was also mailed multiple times), then I was told the doctor was on vacation for 2 weeks, (though it's been 5 weeks since the initial request) During this time, no one else could complete the form except a nurse practitioner whom was not qualified to complete it. Now today after being spoken to disrespectfully and not getting any answers from Shelly who answered the phone, I call back and request a manager, getting Rosana, who says I could have had the form completed and signed by another physician the whole time. I'm now sending to her and I hope to get this done this time. I am more than frustrated and my family situation is suffering because I cannot get this form signed.

Beyond thankful for InnovAge and the care they have provided for my loved one over the years. Seriously would not know what to do with them. Our whole family is so grateful to them all!
